Therefore, we assumed that more use-ful disaster prevention town development activities would be possible by collaboration between Mashiki Town, which has experienced disasters, and the Tokai area, which is preparing for future disasters, and we developed a disaster prevention map creation support system. we engaged in disaster prevention community development activities through collaboration between the two regions using this system. The support system is a system that users use in WS to create digital versions of maps. This support system is aimed at local resi-dents, neighborhood associations, governments, etc., and has an easy-to-understand, easy-to-use interface de-sign and operability so that children can easily use it. In addition, the support system can be used not only on PCs but also on tablet devices such as iPads and mobile devices such as smartphones, so that you can create maps on the spot while walking around town. It can be posted as is on the map of the support system.我が国の高齢化率は2025年には30%を超えると試算され,高齢期の生活は地域包括ケアシステムの構築による支援が推進されている.最期まで安心して暮らすことができる,柔軟性,融通性,汎用性の高い地域デザインについて,地域ごとに検証することは重要な課題である.本研究の目的は,ビッグデータの一つである診療報酬データ(以下,レセプトと表現する)を分析し,退院支援を受けた患者の診療報酬算定内容から,退院支援を要する国民の健康課題の特徴と,退院後利用している医療,介護サービスの利用状況を定量的,地理的な観点で可視化することである.さらに健康課題を有する国民(特に高齢者)の退院後,地域での生活継続性を実現する方策や,地域ごとに不足している社会資源を明確化し,地域の実情ごとに最期まで安心して暮らすことのできる地域デザインを検討する.It is estimated that the aging rate in Japan will exceed 30% by 2025. Furthermore, measures to realize the continuity of life in the community after hospital discharge for people with health problems (especially the elderly), and clarification of social resourc-es that are lacking in each region, will be examined to design a community where people can live with peace of mind until the end of their lives, depending on the actual conditions of each region.
